To apply for Spokane Master's Commission, please follow the instructions below.  All of these steps must be completed
before your application can be processed.

1. Fill out our online application form.
    
2. Print these reference forms and have them filled out according to their instructions:
   • Pastoral Reference Form
   • Confidential Reference Form
      
3. Send the following items by mail after you've submitted the online application form:
   • Your personal testimony (must be typed and a minimum of 300 words)
   • A recent photo of yourself (it will not be returned)
   • A $35.00 application fee (check made out to Spokane Master’s Commission)
